Well I think that covers most of them. Well, except for that limit which cut off about 20 other forces.

Yuan Shu was actually a talented general and well revered by men. He had the services of Sun Jian, whilst his line separated from Shu he was the original "master" of Wu.
Liu Yan was an official looking to escape the war and politics of the North. He fled south and founded his own territory of the backwater lands of Yizhou. He had 4 sons. All of them died except for one in his rebellion. He set up the Ruler Zhang Lu as a buffer state. His son was actually a strong ruler. Plagued by indecision. He thought he could trust a cousin, how he was proven wrong.
Liu Yu was the Governor of You province, he was the Lord of Gongsun Zan. He was revered as a wise man. A Kind ruler. He entrusted such faith in men that he was considered for Emperor when Yuan Shao started his coalition, which was mainly the idea of Cao Cao but still he came to power. Liu Yu did not want to go against the Han court so abstained from fighting. He did offer some minor help. Han Fu [force not there...] was the lord of the entire Ji province and Yuan Shao his underling wanted Liu Yu to be emperor. He refused. They wanted to give him the rank that would allow him to give out ranks like that of an emperor. he refused again and stated that he would flee north if they persisted. [sometimes I wish he did flee] Later he had a falling out with Gongsun Zan. He was then reported to have ambitions to be emperor by Gongsun Zan. He was then captured and killed by Gongsun Zan who cracked a cruel jock saying he would spare Liu Yu if the it would rain. However it was in the middle of the dry season and it did not rain. He was then executed. A folk tale said the heavens began to fill with clouds at his humor but Liu Yu was killed before it rained and then the clouds dispersed. However it was a folk tale. Though all sources state is it did not rain.
Liu Dai was the original owner of what would be Cao Cao's land. After his death Yu Jin gave the province over with that of Li Qian to Cao Cao. Cao Cao avenged him and that was it.
Liu Yao was actually Liu Dai's younger brother. He was an extremely talented civil official but did not know how to lead men. He did not use Taishi Ci's advice and then was conquered. After his fall there was nothing to stop Sun Ce's advance. Nothing tangible.
Liu Du was a distant relative of Liu Biao. He ended up taking a small portion of land in the southern area of Jing. He was conquered by Liu Bei and nothing left is heard of him.
Ma Teng is not a loyal man. He rebelled against the Han. Supported Dong Zhou after his first unsuccessful battle. He served Cao Cao again for rank and was killed 1 year after Ma Chao's rebellion. He was kept alive during this period and insulted Cao. He didn't die because of mal intent. He didn't even die because his son rebelled. He died from stupidity.
Han Sui was a rebel along with Ma Teng. They fought bitterly at times and swore an oath together at others. Supported his son in rebellion but was then betrayed. He continued his fight for 10 years after Ma Chao's defeat. He considered going to Shu but was died before hand. Yan Xing his officer then went to Wei.
The Five Pecks of Rice was a Taoist Sect founded by Zhang Lu's father. He was a prominate warlord and served under Liu Yan. Liu Zhang killed Zhang Lu's mother and wife and caused the bad blood. Zhang Lu was a kind benevolent ruler. He surrendered to Cao Cao and was allowed to continue his sect. It lasted many many years becoming a state religion of one of the minor warlords. His direct line still is important to Taoism to this day.
Zhang Ju was a rebellion that happened shortly after the Yellow Turbans. It failed with Liu Yu and Gongsun Zan conquering them.
Ou Xing lead a rebellion and called himself King. Sun Jian put it down.
Guan Hai lead a force after the Yellow Turban Rebellion. He attacked Kong Rong. He is over dramatized in the Novel. He died shortly after.
Liu Pi and Gong Du were the last of the Yellow Turbans to be conquered. They supported Liu Bei in his flight south. Surrendering Ru Nan to him. They were killed defending Liu Bei from Cao Cao.
Gongsun Du was a commander in the north. He controlled Liao Dong, he controlled a portion of Qing and called it Ping. He destroyed a Korean Kingdom.
Kong Rong and Kong Zhou. They were alike in that they were decedents of Confucius. And that they were worthless in leading an army. Kong Rong fought against Yuan Tan and lost. Kong Zhou fought against minor people and lost. They both surrendered to Cao Cao when he had the emperor.
Gongsun Zan was a tyrant and swore an oath of brotherhood with three wicked people. He killed all his children and caused his own destruction at the hands of Yan Rou and Yuan Shao. Yan Rou was a former officer of Liu Yu.
Tao Qian was sick and gave the province to Liu Bei but his officer Wang Lang lead a force to Yang and made his own little force they were close. Wang Lang could be said to be the direct force to follow Tao Qian after all Liu Bei was given the province by Sun Qian and Mi Zhu and Chen Gui.
